eSalvar Privacy Statement

Naga City COVID-19 Contact Tracing Initiative

eSalvar is a CoVID-19 contact tracing system of the City Government of Naga, which requires commercial establishments and government offices to scan the QR ID's of individuals entering/exiting their premises. eSalvar does not require individuals to have a smartphone. It does not use Bluetooth and geo-location technologies. eSalvar is compliant with the Philippines Data Privacy Act of 2012 (R.A. 10173). WHAT DATA IS COLLECTED? AND FOR WHAT PURPOSE?

When an Establishment Registers for an Establishment ID

  1. Complete Name (as registered with the City Government of Naga)

  2. Short Name / Popular Name

  3. Business Identification Number (indicated in business permit)

  4. Complete Address

  5. Barangay

  6. Name of Representative

  7. Role or Designation of Representative

  8. Mobile Number of Representative

  9. Email Address of Representative

These are collected for the purpose of verifying the identity of the establishment with the records of the City Government of Naga and when verified, for granting of an Establishment ID (plus username and password) to be used to login eSalvar app.

When an Individual Registers for a QR ID

Personal Information:

  1. First Name

  2. Middle Name

  3. Last Name

  4. Suffix (optional)

  5. Birth Date

  6. Gender

  7. Profession/Occupation (optional)

  8. Nationality

  9. Mobile Number

  10. Profile Photo

Permanent Address:

  1. No/Door/Unit, Apartment/Building

  2. Lot Number

  3. Block Number

  4. Phase Number

  5. Street/Avenue

  6. Zone Number

  7. Subdivision

  8. Country

  9. Province

  10. City, Town, or Municipality

  11. Barangay

These are collected for the purpose of administering your registration and generating an individual QR ID. These are also processed for the purpose of CoVID-19 contact tracing.

When an Individual Checks-In/Out of Establishment

  1. Individual QR Code

  2. Date of Check-In/Out

  3. Time of Check-In/Out

  4. Temperature (required if 37.5 degrees celsius or higher)

  5. Establishment ID

These are collected for recording individual check-in/out on establishments and are processed for the purpose of CoVID-19 contact tracing.

 

WHY PROCESS PERSONAL DATA? ON WHAT LEGAL BASIS?

We process personal data only for the purpose of CoVID-19 contact tracing in accordance with the Philippine Data Privacy Act of 2012 (R.A. 10173) and the Naga City Ordinance No. 2020-086 on the enforcement of eSalvar available at this link — http://naga.gov.ph/wp-content/uploads/2010/06/ord2020-086.pdf

When you register, we ask you to authorize:

“I hereby authorize the Health Emergency Response Task Force (HERT) of Naga City to collect and process my personal information and sensitive personal information (collectively, “Personal Data”) as stated above for the purpose of effecting control of the COVID-19 infection.  I understand that my personal data is protected by the Data Privacy Act of 2012 (R.A. 10173) and that I am legally required to provide truthful information pursuant to the Bayanihan to Heal as One Act (R.A. 11469), as may be amended, and other government issuances relative to COVID-19 prevention and control."

When you register, we ask you to attest:

“I further attest that I have provided the above information to the best of my personal knowledge. I have not withheld any relevant information or given false information regarding the foregoing. By signing below, I further consent to the processing including, but not limited to, the collection, use and disclosure of my above-declared personal data for the purposes of contact tracing relative to COVID-19. All personal data collected for these purposes shall be retained only for a period allowed by existing  government regulations and issuances. After which, all personal data will be disposed of in a secure manner that would prevent further processing and/or unauthorized access or disclosure.”

 

HOW WE COLLECT DATA?

Data is collected when an individual registers for a QR ID; and when an individual checks-in/out of establishment (please refer to #1C for details). The camera in the eSalvar App does not take a picture of the Citizen QR ID nor does it and/or the app save the Citizen QR ID as an image in the photo gallery or device storage. The camera is ONLY used to scan the QR code portion of the Citizen QR ID to extract its “alphanumeric/symbolic string” equivalent for processing. These data collected are encrypted when transferred to the secure servers of the City Government of Naga for use in CoVID-19 contact tracing.

 

HOW WE PROTECT YOUR DATA?

Data is protected is various ways and aspects from system design, organizational measures and technical procedures. These include: a secure algorithm for generating the QR ID of individuals, which data cannot be interpreted outside of the system; a one-time PIN (OTP) to verify individuals during registration; and an access to the system restricted only to the contact tracing team of the Naga City Health Emergency Response Task Force (HERTF) of the City Government of Naga. Further, establishments  can only scan your QR ID but they have no access to your personal information and check-in/checkout history.

 

WHEN DO WE SHARE YOUR DATA?

The only time your data is shared is when the system determines that you may have been in contact with someone who tested positive for CoVID-19 as a result of a contact tracing activity. Your personal information is shared with government and enforcement agency so that they can render you timely and appropriate health precautions and intervention.

 

DO WE RETAIN YOUR DATA? HOW LONG?

We retain your personal data for as long as eSalvar remains an active program of the City Government of Naga in addressing the CoVID-19 pandemic. Only the past thirty (30) days your check-in/out records are retained in the system. Your check-in/out records beyond 30 days are automatically deleted in the system.

 

ACCESS, CORRECTION AND DELETION OF PERSONAL DATA

You have the right to access your personal information registered with us. If for some reason you have registered an incorrect information with the system and wants it to be corrected, we encourage you to do so.
